Supervisees must submit a Supervision Agreement Form within thirty (30) days of application or employment. If there is a change in supervisors, the Supervisee must complete another Supervision Agreement Form and turn it in to the Board within thirty (30) days of such change. Licensees may only be supervised by a Speech-Language Pathologist or Audiologist who holds a current, unsuspended, unrevoked license in accordance with Act 892.

Licensees should complete all supervision requirements consistent with the license held and immediately thereafter submit appropriate supervision forms to the board office along with a written request for a license upgrade. Supervisees must remain in supervision until their license is upgraded!
