Question: Since I didn't receive the required direct/alternative supervision hours one month, may I make up the hours next month?

Answer: No. According to Rule No. 121D a minimum of on-site, in-view supervision hours must occur each week for a Provisional Assistant or Assistant Speech-Language Pathologist. The Board requires that the Licensee complete additional months of supervision to cover the months of incomplete supervision.

 

Question: When do I submit my Continuing Education (CE) hours if I take "retired status?"

Answer: When a "retired" individual returns to active status, he or she must submit five hours of CE for each year that retired status was claimed. CE hours are due June 30 of the year active status is renewed. As an alternate five hours may be submitted each year at renewal time while in "retired" status.

 

Question: What happens if I'm late renewing my license?

Answer: Renewals are due by June 30 each year. The following penalties are added according to the tardiness of the renewal:

a. July 1-31: The fee is raised to $100.

b. August 1-October 31: The fee is raised to $200.

C. After October 31, the license is lapsed. To reinstate with a new application prior to the following June 30, the fees are: application fee $100, delinquency fee $200.

 

Question: If I let my Restricted Speech-Language Pathology License lapse, can I get it back? Is there a time-limit?

Answer: A Restricted License may be reinstated. There is no time limit.
